29 CFR  1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times.  VTC room, Bldg. 214: On or about October 1st, 2024 at times prior thereto, a ceiling panel in the VTC room was improperly installed. This condition would delay the activation of the sprinkler system(s) preventing a proper fire/smoke barrier and restricting the movement of smoke in accordance with NFPA 101 8.5.1. This condition potentially exposed employee(s) to fire and smoke inhalation hazard(s).

Safety Office, Room 127, Bldg. 214: On or about October 1st, 2024, and at times prior thereto the employer did not ensure at least 3 feet in front of the electrical panel was clear. The circuit breaker panel had two (2) storage racks, two (2) vacuum cleaner(s), multiple boxes and computer monitor(s) obstructing the panel. This condition interferes with access and immediate shutdown of a circuit in the panel, particularly in the event of an emergency. This condition potentially exposed employee(s) to electrical shock and/or fire hazard(s).

29 CFR  1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket).  On or about October 1st, 2024, and at times prior thereto employee(s) used the TASKVAC commercial vacuum cleaner to vacuum the office spaces. Also, employee(s) used the Belkin 7-Outlet SurgeMaster Surge Protector to plug a Levoit True HEPA Air Purifier and a WOOZOO Fan Circulator. The TASKVAC commercial vacuum cleaner and the Belkin 7-Outlet SurgeMaster Surge Protector  was not visually inspected for exposed wiring. This condition potentially exposed employee(s) to electrical shock and/or fire hazards.  In the following location(s): a) Safety Office, Room 127, Bldg. 214 - TASKVAC commercial vacuum cleaner. b) IT Emergency Management Office, Room 125, Bldg. 214 - Belkin 7-Outlet SurgeMaster Surge Protector.
