Wynn Rd. Oregon, Ohio 43616: The employer did not ensure open floor holes were effectively guarded or covered on the Manual Line. Due to this condition, employees operating the Manual Bruno press in proximity of these holes were exposed to a tripping/falling hazards.

Wynn Rd. Oregon, Ohio 43616: The employer did not effectively train employees to ensure that the purpose and function of the energy control program was understood by employees. Due to this condition employees tasked with cleaning heat tables one and two of the Turbo (Rat) Line were exposed to crushing hazards when performing service and maintenance tasks.

Wynn Rd. Oregon, Ohio 43616: The employer did not ensure employees were adequately protected from pinch points, caught in hazards, and crushing hazards created by the air actuated clamps of the "Wheel Arch Liner" mold on the Manual line.  Due to this condition, employees tasked with operating the air actuated clamps were exposed to abovementioned hazards.

Due to this condition employees relocating and installing electrical equipment for the Manual Line Contact Ovens were exposed to electrical shock and arch flash hazards.

Oregon, Ohio 43616: The employer did ensure employee performing electrical work on 110v, 220v and  480v systems, were provided with and used appropriate electrical protective equipment for the body, hands, head, and face. Due to this condition employees tasked with installing and maintaining electrical equipment where potential live conductors were present, were exposed to electrical shock and arc flash hazards.
