On or about October 7, 2024, the employer failed to create and implement a written Personal Protective Equipment (PPE) hazard assessment for employees operating the Rat and Cannon lines. This includes spraying mold release also known as HT9129 on the product and pulling product from the line that have been sprayed with Elastoflex 28000R Resin and Specflex NS 1540 Isocyanate DA that contain respirable hazards including but not limited to 4,4-diphenylmethane diisocyanate.
