29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: Warehouse, 165 Flanders Road, Westborough, MA: On October 2, 2024, and at times prior, employees operated three Yale fork trucks, model # ERP040VFN36TE078, identification numbers 608, 703, and 704 that were equipped with a clamp attachment that was not factory installed. The data plate on the trucks showed 43-inch standard forks as the approved attachment, however, a clamp attachment was installed and being utilized. The truck data plates were not marked to identify these clamp attachments and did not show updated truck weight and load capacities.
