The employer did not ensure eye protection was utilized by employees operating pneumatic nail guns. Employees using nail guns without eye protection were exposed to struck-by hazards from broken wood, roofing materials, or nail debris.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access: On October 2, 2024, Top Dry Roofing LLC was re-roofing a residential building located at 230 West 4th Street in Wentzville, Missouri. The employer did not ensure an extension ladder placed on the East side of the building above the entrance door awning was properly positioned. The ladder side rails did not extend three feet past the upper landing surface, exposing employees to fall hazards.
