On or about 6/23/2025, the employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record each employee who were engaged in residential roofing operations 6ft (1.8m) or more above lower levels. The employees were not trained in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury while at the job site located at 3837 McClay Road, Saint Peters MO. The employees were conducting roofing operations that exposed them to the hazard of falling from a heights greater than ten (10) feet.
