The press was being operated while the rear gate door was open and the interlock on the rear gate is malfunctioning and/or not fully connected for use.

Light curtains that were in place were not functioning properly and at times bypassed, and a two hand pendant control was utilized that was not affixed to the ground and with the press in continuous mode.
