29 CFR 1926.1427(d)(4): An employer permitted an operator to operate equipment with a certification card issued more than 5 years prior to the date of operation. a) On or about October 1, 2024, at site address 150 Imperial Blvd., CAPE CANAVERAL, FL 32920: employees were exposed to struck-by hazards, in that the crane operator using the Palfinger Crane Truck (PW 42001-SH Serial No. 100743984) to lift drywall certification had expired on 07/29/2023.

29 CFR 1926.501(b)(1):Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest system. a) On or about October 1, 2024, at the worksite, the employer did not enforce the use of fall protection for employees delivering construction materials to a 6th floor, exposing employees to an approximate 60 ft. fall hazard. The Interior Exterior Building Supply was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1648844, citation number 1, item number 1 and was affirmed as a final order on 04-27-2023, with respect to a workplace located at 609 St. Francis Parkway, Gonzales, LA 70737.
