29 CFR 1910.147(c)(7)(i):The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: a) Unverferth Manufacturing Co Inc. on or about October 03, 2024, at the address of 601 S. Broad St. Kalida: The employer did not ensure that employees who setup the 230-ton Cincinnati hydraulic press brake (serial # 54715) were trained in the skills required for the safe application, usage, and removal of energy controls. Employees were not trained to effectively isolate and render energy controls safe prior to performing setup activities and as a result were exposed to pinch points.

Broad St. Kalida: Employees changing dies on the 230-ton Cincinnati hydraulic press brake (serial #: 54715) were exposed to caught in injuries in that the employer did not utilize established procedures to verify and control the hazardous energy created by the moving components of the Hydraulic press brake # 1088.
