29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: a) On October 3, 2024, at the above addressed jobsite, employees working an excavation of up to approximately eight (8) feet and three (3) inches in depth, were not provided a suitable means of egress. Lenny Hoffman Excavating, Inc. was previously cited for a violation of this occupational safety and health standard 29CFR 1926.651(c)(2), which was contained in OSHA inspection number 1760161, citation number 1, item number 1 and was affirmed as a final order on August 23, 2024, with respect to a workplace located at 230 Green Bay Road in Glencoe, Illinois. No abatement certification or documentation is required for this item.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). a) On October 3, 2024, at the above address, employees were exposed to cave-in hazards while performing work in an unprotected excavation of approximately seven (7) feet two (2) inches in depth. The employer did not ensure that employees were protected from cave-in hazards. Lenny Hoffman Excavating, Inc. was previously cited for a violation of this occupational safety and health standard 29CFR 1926.652(a)(1), which was contained in OSHA inspection number 1683713, citation number 1, item number 1 and was affirmed as a final order on August 7, 2023, with respect to a workplace located at 865 Warwick Road in Deerfield, Illinois.
