29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: a) Front Upcut Saw Area - On or about September 16, 2024, an employee was exposed to flying aluminum chips when operating a Front Upcut Saw (Aslan Machine Inc. - US1 -700; Serial # US-1-700-18010815) due to not wearing safety goggles for eye protection. b) Rear Upcut Saw Area - On and prior to November 26, 2024, an employee was exposed to flying aluminum chips when operating a Rear Upcut Saw (Aslan Machine Inc. - Serial # US1-700-22010816) due to not wearing safety goggles for eye protection.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Front Upcut Saw Area - On or about September 16, 2024, an employee was exposed to flying aluminum chips when operating a Front Upcut Saw (Aslan Machine Inc. - US1 -700; Serial # US-1-700-18010815) without the Blue Guards in place. b) Machine Shop - On and prior to October 3, 2024, employees were exposed to flying aluminum chips when operating a milling machine (Bridgeport - Serial No. J27279 (yellow handle)) without guarding in place. c) Machine Shop - On and prior to October 3, 2024, employees were exposed to flying aluminum chips when operating a milling machine (Bridgeport (red handle)) without guarding in place. d) Machine Shop - On and prior to October 3, 2024, employees were exposed to flying aluminum chips when operating a milling machine (PM-1054TV) without guarding in place. e) Machine Shop - On and prior to October 3, 2024, employees were exposed to flying aluminum chips when operating a milling machine (PM-950V) without guarding in place.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical: a) Front Upcut Saw Area - On and prior to November 26, 2024, employee operated a Front Upcut Saw (Aslan Machine Inc.) that utilized a chemical (Coolube 2210) without the container being labeled. b) Rear Upcut Saw Area - On and prior to November 26, 2024, an employee operated a Rear Upcut Saw (Aslan Machine Inc. - Serial # US1-700-22010816) that utilized a chemical (Coolube 2210) without the container being labeled.
