29 CFR 1926.501(b)(11):"Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. a) On or about October 4, 2023, the employer failed to ensure employees working on a steep roof were protected from fall hazards. Specifically, while removing old roofing material and installing new roofing material on a roof with a 6:12 pitch, employees were exposed to fall hazards greater than 25 feet due to the absence of a fall protection system.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components shall be inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity. a) On or about October 4, 2024, the employer failed to conduct inspections of the scaffolding for visible defects as required by a competent person before each work shift. The lack of inspections exposed employees to the potential risk of a fall hazard of 25 feet while working from the frame scaffolding.
