29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about October 07, 2024, the employer did not provide a training program for each employee exposed to a fall hazard of 14 feet above concrete while replacing shingles on the roof of a commercial building located at 1562 Akron Peninsula Road, AKRON, OH 44313.

29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  On or about October 7, 2024, an employee was exposed to a struck by hazard while working 14 feet below an active roofing crew at a commercial building located at 1562 Akron Peninsula Road, AKRON, OH 44313, and was not provided a protection helmet by the employer.  A-1 Roofing & Siding was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.100(a), which was contained in OSHA inspection number 1530656, citation number 1, item number1 and was affirmed as a final order on 6-17-2021, with respect to a workplace located at 206 Lake Ave, Massillon, Ohio 44646.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   On or about October 07, 2024, employees were exposed to a fall hazard of 14 feet above concrete while removing shingles from a commercial roof located at 1562 Akron Peninsula Road, AKRON, OH 44313. Each employee was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  A-1 Roofing & Siding was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1530656 citation number 1 item number 2 and was affirmed as a final order on 06-17-2021, with respect to a workplace located at 206 Lake Ave. Massillon, Ohio 44646.
