29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). (a) On or about October 8, 2024, employees conducting roofing work on the upper two-story portion of an existing roof with a 6:12 pitch and 10:12 pitch, were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 18 feet over the front of the residential property. (b) On or about October 8, 2024, employees conducting roofing work on the lower garage portion of an existing roof with a 6:12 pitch, were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 9 feet over the front of the residential property.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) On or about October 8, 2024, the employer did not ensure the portable extension ladder used by employees to access the upper two-story portion of the residential home extended at least three feet above the upper landing, exposing employees to a fall hazard of approximately 18 feet to the ground below. (b) On or about October 8, 2024, the employer did not ensure the multipurpose ladder used by employees to access the garage roof of the residential home extended at least three feet above the upper landing, exposing employees to a fall hazard of approximately nine (9) feet to the driveway below.
