29 CFR  1910.178(m)(5)(i): When a powered industrial truck is left unattended, load engaging means shall be fully lowered, controls shall be neutralized, power shall be shut off, and brakes set. Wheels shall be blocked if the truck is parked on an incline.   a) On or about October 8, 2024, the employer did not ensure employees did not leave the forklift unattended,  in that employees were exiting the running forklift to obtain the hook located approximately 28 feet away and did not have the forklift in view.

When the operator of an industrial truck is dismounted and within 25 ft. of the truck still in his view, the load engaging means shall be fully lowered, controls neutralized, and the brakes set to prevent movement.   a) On or about October 8, 2024, the employer did not ensure that the load engaging means were fully lowered prior to dismounting the industrial truck, in that employees dismounted the industrial truck while the load remained raised to disengage the hook to allow the load to dump from the hopper.

29 CFR  1910.217(e)(1)(i): Inspection, maintenance, and modification of presses -Inspection and maintenance records. The employer shall establish and follow an inspection program having a general component and a directed component.   (A) Conduct periodic and regular inspections of each power press to ensure that all of its parts, auxiliary equipment, and safeguards, including the clutch/brake mechanism, antirepeat feature, and single-stroke mechanism, are in a safe operating condition and adjustment;  (C) Maintain a certification record of each inspection, and each maintenance and repair task performed, under the general component of the inspection program that includes the date of the inspection, maintenance, or repair work, the signature of the person who performed the inspection, maintenance, or repair work, and the serial number, or other identifier, of the power press inspected, maintained, and repaired.  a)  On or about October 30, 2024, the employer did not ensure that annual inspections were conducted on the Minster presses 2601 and 2602 to ensure that modifications made to the presses were in safe operating condition.  b) On or about October 8, 2024, the employer did not ensure that maintenance records for the Minster Presses (2601, 2602, and 2603) and the Seyi 330T 0201 press included the signature and correct name of the employee performing the work.

29 CFR  1910.217(b)(7)(iii): Mechanical power press guarding and construction, general - Machines using part revolution clutches. A means of selecting Off, "Inch," Single Stroke, and Continuous (when the continuous function is furnished) shall be supplied with the clutch/brake control to select type of operation of the press. Fixing of selection shall be by means capable of supervision by the employer.  a) On or about October 8, 2024, the employer did not control the fixing of selection between off, inch, single stroke, and continuous for the Seyi SAG 330 ton West (D300-103) part revolution press in that the selector key was left in the press during operation.  b) On or about October 30, 2024, the employer did not ensure the 2602 minster had the means of selecting off, "inch", "single stroke", and "continuous."   First Brands Group, LLC dba Fram Group Operations, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.217(b)(7)(iii), which was contained in OSHA inspection number 1710130, citation number 1, item number 3 and was affirmed as a final order on May 24, 2024, with respect to a workplace located at 851 Jackson St, Greenville, OH 45331.

The employer shall train and instruct the operator in the safe method of work before starting work on any operation covered by this section. The employer shall ensure by adequate supervision that correct operating procedures are being followed.  a) On or about October 8, 2024, the employer did not ensure that employees operating the Seyi 330 ton press (D300-103 West) were adequately trained to understand that the purpose of supervisory control of the selector switch, in that the selector switch key was kept in the control panel during production.   b) On or about October 30, 2024, the employer did not ensure that employees operating the Minster presses 2601 and 2602 were instructed in the safe method of work after modifications were made to the press that included the removal of the selector switch, and the addition of the new control panel, PLC (including the addition of safety input, and door guard switches).

29 CFR  1910.147(c)(4)(ii): General - Energy control procedure. The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  (B) Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; (C) Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and (D) Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.  a) On or about December 14, 2024, the employer did not ensure that the energy control specific procedures for Minster presses 2602, and 2603 located in the 2 and 5/8ths department were correct in that:  1.  It did not include the correct identification for the E1 identified on the south wall panel #406, in that it did not identify the panel as the energy isolation point for presses 2601, 2602, and 2603.   2.  It did not identify the isolation point for the pneumatic energy for each of the presses, in that the only isolation point identified was the A1 for press 2602, and not the pneumatic energy isolation points for presses 2601 and 2603.   3. It did not identify the hydraulic energy isolation point that powered the hydraulic pumps.  4.  It did not  identify machine specific energy isolation points for each piece of auxiliary equipment, such as  the conveyor, the roll feed, the oiler, and the uncoiler.
