1910.303(e)(1) Identification of manufacturer and ratings. Electric equipment may not be used unless the following markings have been placed on the equipment:  (ii): Other markings giving voltage, current, wattage, or other ratings as necessary.  a.) On October 30, 2024, the employer did not ensure that the 3 electrical panels connected that were located on the south facing wall of the 2 and 5/8's department had identification of the manufacturer and ratings of voltage, current, wattage and other ratings as necessary on the panels.  These unlabeled electrical panels were identified as the energy isolation devices for the Minster presses 2601, 2602 and 2603 by company officials.  They were not the electrical isolation point for the presses.   b.) On October 30, 2024, the employer did not ensure that the dark gray electrical panel on the south facing wall of the 2 and 5/8's department was legibly marked to indicate that it was the energy isolation point for the 3 Minster presses (2601, 2602, and 2603., nor was there other markings giving the voltage, current, wattage or other ratings for the electrical panel.

29 CFR  1910.145(a)(1): These specifications apply to the design, application, and use of signs or symbols (as included in paragraphs (c) through (e) of this section) that indicate and, insofar as possible, define specific hazards that could harm workers or the public, or both, or to property damage. These specifications are intended to cover all safety signs except those designed for streets, highways, and railroads. These specifications do not apply to plant bulletin boards or to safety posters.  a) On or about October 30, 2024, the employer did not ensure that the three electrical panels identified for the Minister Presses  located on the south wall of the 2 and 5/8th's department were labeled with signs or symbols to identify the potential electrical hazards.

1910.303(b)(1) Examination. Electric equipment shall be free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined using the following considerations:  (i):Suitability for installation and use in conformity with the provisions of this subpart; Note to paragraph (b)(1)(i) of this section: Suitability of equipment for an identified purpose may be evidenced by listing or labeling for that identified purpose.  a) On or about October 30, 2024 the employer did not ensure that the following equipment purpose was identified, the electrical disconnect for the mechanical power presses 2601 and 2602 did not have labeling that identified it as the disconnect for the 2601, 2602 and 2603, the control panel for the 2601, 2602, and 2603 did not have labeling  that listed each control panel for each power press, and the electrical panels located next to the electrical disconnect did not have any listing or labeling as to its identified purpose.

29 CFR  1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  a) On or about October 30, 2024, the employer did not ensure employees working with and around the Minster steel coil feeder for press 2602 were protected from amputation hazards such as, but not limited to, the auxiliary equipment such as the Perfecto coiler line and the steel moving during production.  b) On or about February 6, 2025, the employer did not ensure employees working with the Quaker Draw 541 mixer at the Minster steel coil feeder for press 2602 were protected from amputation hazards associated with the unguarded moving mixing drill in the metal lubrication tank.

29 CFR  1910.303(g)(1): Space about electric equipment. Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment.  a) On October 30, 2024 and February 6, 2025 the employer did not ensure the space surrounding the electrical light distribution panel (P1 1Ph 3W 120/240 panel) located on the south wall near the minster presses were kept clear about the electrical panel to permit ready and safety operation and maintenance of equipment.
