29 CFR 1915.74(a)(1): A gangway of not less than 20 inches walking surface of adequate strength, maintained in safe repair and safely secured was not used. If a gangway is not practical, a substantial straight ladder, extending at least 36 inches above the upper landing surface and adequately secured against shifting or slipping was not provided. When conditions are such that neither of gangway nor a straight ladder can be used, a Jacob's ladder meeting the requirements of (d)(1) and (2) were not used. Berth C58, Sea Scout: On October 8, 2024 and times prior thereto, employees accessed the Sea Scout vessel without the use of a gangway, straight ladder or Jacob's ladder by walking over the dock structure and pilings. This condition potentially exposed employees to falls hazards.
