29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) On or about October 9, 2024, an employee was roofing a single-story house more than 6 feet above the ground below without adequate fall protection.  Juan Carlos Salazar was previously cited for a high gravity-serious violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1705752, citation number 1, item number 1, and was affirmed as a final order on 03/13/2024, with respect to a workplace located at 230 Headwater St, Fate, TX 75189.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) On or about October 9, 20234, employees using a 24-foot Werner aluminum extension ladder placed up against the roofs edge on the front/north side of the house with its side rails approximately eighteen (18) inches above the roofline were not protected from falling.  (b) On or about October 9, 20234, employees using a 24-foot Werner aluminum extension ladder placed up against the roofs edge on the back/south side of the house with its side rails approximately eighteen (18) inches above the roofline were not protected from falling.  (c) On or about October 9, 20234, employees using a 24-foot Werner aluminum extension ladder placed up against the roofs edge on the east side of the house with its side rails approximately twenty-four (24) inches above the roofline were not protected from falling.   Juan Carlos Salazar was previously cited for a high gravity serious violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1675348, citation number 1, item number 1 and was affirmed as a final order on 07/11/2023, with respect to a workplace located at 1622 Knoll Crest Dr, Kaufman, TX 75142.  Juan Carlos Salazar was previously cited for a repeat serious violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1705752, citation number 2, item number 2 and was affirmed as a final order on 03/13/2024, with respect to a workplace located at 230 Headwater St, Fate, TX 75189.
