Torres at worksite located at 1415 S. Main Street, Findlay, Ohio, 45840: On or about October 9, 2024, the employer failed to ensure that each affected employee used appropriate eye or face protection when performing re-roofing/sheeting activities atop a commercial structure while exposed to potential struck by hazards (flying particles/debris) while using equipment such as, but not limited to, powered blowing tools and circular saw.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest system.    a) Wtorres Construction Company LLC at worksite located at 1415 S. Main Street, Findlay, Ohio, 45840: On or about October 9, 2024, the employer failed to ensure that employees were protected from falls while performing sheathing and roofing operations atop a single-story commercial structure. Employees installing sheathing without the benefit of fall protection were exposed to falls of approximately 14 feet or greater to the ground below.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee who might be exposed to fall hazards:  a) WTorres Cosntruction Company LLC at worksite located at 1415 S. Main Street, Findlay, Ohio, 45840 : On or about October 9, 2024, the employer failed to ensure that employees had been trained in the appropriate use of fall protection such as, but not limited to the use of personal fall arrest systems or alternate measures while performing operations from elevated surfaces greater than six feet from the ground below.
