29 CFR 1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. At a worksite located at 2100 SW Swope Drive, Independence, MO 64057, employees were exposed to fall hazards. The employer exposed the employees to these hazards by allowing them to perform roofing work at heights greater than 6 feet above the next lower level without the use of fall protection.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device was not provided to assist employees in mounting and dismounting the ladder. At a worksite located at 2100 SW Swope Drive, Independence, MO 64057, employees were exposed to fall hazards. The employer exposed the employees to these hazards by allowing them to use a portable extension ladder whose side rails did not extend at least 3 feet above the level for which it was being used to gain access, nor was the ladder secured to the structure at the top with a grasping device provided for employees while mounting and dismounting the ladder.
