29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  Maintenance: On or about October 15, 2024, and at times prior thereto, the employer had not created a facility specific energy control program, had not conducted training, and had not conducted periodic annual inspections of Lock Out Tagout Out procedures for two different types of pin setting equipment to ensure employee's, prior to performing servicing or maintenance on equipment, understood how stored energy could occur and cause injury, which potentially exposed employees to electrocution, burns, crushing, cutting, lacerating, amputating, and fractures.
