29 CFR  1926.403(b)(1)(iii):Safety of equipment was not determined on the basis of electrical insulation.  At a worksite located at 2115 SW Hook Farm Dr, Lees Summit, MO 64082, employees were exposed to electric burns, shocks, arc flash, and fire hazards.  The employer exposed the employees to these hazards by allowing them to utilize an electric circular saw that had damage to the outer sheathing of its power cable.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At a worksite located at 2115 SW Hook Farm Dr, Lees Summit, MO 64082, employees were exposed to fall hazards.  The employer exposed the employees to these hazards by allowing them to utilize a portable extension ladder while the side rails did not extend at least 3 feet above the level to which it was being used to gain access.

The employer exposed the employees to these hazards by allowing them to work in an area where there was a possibility of head injury from falling objects without using appropriate head protection.   Ludwin Urbina DBA LMW Contracting LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.100(a), which was contained in OSHA inspection number 1681276, citation number 1, item number 1 and was affirmed as a final order on 11-22-2023, with respect to a workplace located at 17028 Lucille St., Overland Park, KS 66221.   Urbina General Contracting, LLC.  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.100(a), which was contained in OSHA inspection number 1521484, citation number 1, item number 1 and was affirmed as a final order on 7-14-2021, with respect to a workplace located at 2 Dakota Circle, Lake Winnebago, MO 64030.

The employer exposed the employees to these hazards by allowing them to utilize an electrical extension cord with strain relief damage to power an electric circular saw.   Urbina General Contracting LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.405(g)(2)(iv), which was contained in OSHA inspection number 1479599, citation number 1, item number 1 and was affirmed as a final order on 7-27-2020, with respect to a workplace located at 15900 W 96th St, Lenexa, KS 66227.
