29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee was using eye or face protection when exposed to eye or face hazards from flying particles.  At a residential worksite located at 205 Kansas City Ave S, Excelsior Springs MO -- employees engaged in tearing off a pre-existing roof using pitchforks, hammers and other small hand tools were not wearing eye protection. This exposed the employees to getting shrapnel and other debris in their eyes causing injury. This was documented on or about 10/17/2024.  Barrientos Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1734062, citation number 1, item number 1 and was affirmed as a final order on 5/21/2024, with respect to a workplace located at 1323 Highland Ave, KC MO.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   At a residential worksite located at 205 Kansas City Ave S, Excelsior Springs MO -- employees engaged in roofing duties tearing off the pre-existing roof were not wearing/using any personal fall arrest system (fall protection). This exposed the employees to fall hazards greater than 14' above ground level. This was documented on or about 10/17/2024.  Barrientos Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1734062, citation number 1, item number 2 and was affirmed as a final order on 5/21/2024, with respect to a workplace located at 1323 Highland Ave, KC MO.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At a residential worksite located at 205 Kansas City Ave S, Excelsior Springs MO -- employees engaged in roofing duties used an extension ladder to gain access to the roof and descend from. The ladder did not extend 3 feet above the landing surface (the roof). This exposed employees to fall hazards 9' above deck level. This was documented on or about 10/17/2024.  Barrientos Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1734062, citation number 1, item number 3 and was affirmed as a final order on 5/21/2024, with respect to a workplace located at 1323 Highland Ave, KC MO.
