29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): Building 284 - On or about October 18, 2024, the fire alarm system was in a state of trouble due to a system fault, the employer was silencing the fault to stop the fault alarm from making the noise, this bypass required that enclosure be removed exposing employees to live parts, no temporary shielding was installed around the exposed parts operating above 50 volts. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1960.8(c): 29 CFR 1960.8(c): The Agency did not develop, implement, and evaluate an occupational safety and health program in accordance with the requirements of section 19 of the Act, Executive Order 12196, and the basic program elements prescribed in this part, or approved alternate program elements: a. Building 284 - On or about October 18, 2024, the employer failed to implement DODI 6055.1 enclosure 3 section 8, b. for documenting and notifying effected staff of an unsafe condition in the workplace and implementation of Risk Management strategies. The employer had been aware of fault in the fire alarm system that required bypassing the audible chirping which meant exposed parts were unguarded to prevent exposure. Employees and supervisory personnel were exposed to electrical hazards. b. Building 284 - On or about October 18, 2024, the employer failed to develop and implement an emergency action plan/ occupant evacuation plan through the incorporation of NFPA 101 in compliance with DODI 6055.06 section 6 and DCMA Manual 4201.16 section 14.1d(4). c. Building 284 - On or about October 18, 2024, the employer failed to develop and implement a site-specific safety and health program developed and tailored to the agency establishment and operations conducted at DLA Susquehanna in accordance with DODI 6055.1 through incorporation of DCMA HQ's manual - DCMA Manual 4201.16. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
