29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces are inspected, regularly and as necessary, and maintained in a safe condition: (a) On or about October 18, 2024, and at times prior, the employer exposed employees to trip and tip over hazards, in that, the employer did not inspect and maintain warehouse floors in a safe condition for employees manually handling and operating forklifts to transport materials. Forklift traffic damaged and created loose concrete on the floor surface.

29 CFR 1910.22(d)(2):29 CFR 1910.22(d)(2): The employer did not ensure that hazardous conditions on walking-working surfaces are corrected or repaired before an employee uses the walking-working surface again. The employer did not ensure that if the correction or repair cannot be made immediately, the hazard is guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired. (a) On or about October 18, 2024, and at times prior, the employer exposed employees to trip and tip over hazards, in that, the employer did not repair or guard warehouse floors that had been significantly damaged from forklift traffic while employees manually handled and operated forklifts to transport materials.
