29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:   At the temporary jobsite located at 16884 Cardinal Street Olathe, Kansas, employees engaged in residential framing activities were exposed to electrical shock and fire hazards in that the extension cords used to supply power from electrical box to power tools were frayed.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   At a temporary location at 16884 Cardinal Street Olathe, Kansas, employees engaged in residential framing activities were exposed to fall hazards of approximately 21 feet 11 inches from the top edge of the second level of the structure to the ground below in that fall protection was not utilized.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  At the jobsites located at 16884 Cardinal Street Olathe, Kansas, employees engaged in residential framing activities were exposed to fall hazards of approximately 21 feet 11 inches from the roof to the ground below, in that the aluminum ladder used to access the roof did not extend at least 3 feet above the eve of the roof.
