29 CFR 1910.184(c)(9): Employee(s) were not kept clear of load(s) suspended, or about to be lifted by sling(s): (a) 604 Hemlock Road, Morgantown, PA - On or about October 8, 2025, employee(s) who were painting large metal sections which weighed between 3,000 to 5,000 pounds were not protected from a crush hazards while working next to or under the suspended or partially suspended load. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

(a) 604 Hemlock Road, Morgantown, PA - On or about October 16, 2024, the employer did not report the left middle fingertip amputation within 24-hours of the amputation incident which occurred on October 11, 2024. No abatement, certification, or documentation required.
