29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible: a) Worksite, Moulder Machine, Hopper: On or about October 17, 2024, an employee was removing cheese from the augers within the hopper of the 4 Screw Moulder Machine (Model: M50C4-E, S/N 003201) and the guard for the augers was not affixed to the machine. The employee sustained a lower arm amputation.
