29 CFR  1926.102(a)(1):Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:  At the jobsite located at 606 Sweetwater Circle, Maize KS, employees engaged in residential re-roofing activities with the use of pneumatic nail guns were exposed to struck-by hazards, in that eye protection was not utilized.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  At the jobsite located at 606 Sweetwater Circle, Maize KS, employees engaged in residential roofing activities were exposed to fall hazards of approximately 9 feet 5 inches from the eave to the ground below in that fall protection was not utilized.

29 CFR  1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.  At the jobsite located at 606 Sweetwater Circle, Maize KS, employees engaged in residential roofing activities were exposed to fall hazards of  approximately 9 feet 5 inches  by not having the Keller K-Pro adjustable ladder properly adjusted to the required 3' height above the landing surface.
