29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about October 24, 2024, employees were exposed to a fall hazard of 10.2 feet while completing roofing work on residential home located at 6483 Bayberry Drive, SEVEN HILLS, OH 44131. Employees were using an extension ladder to access the roof, and the employer did not ensure it extended at least 3 feet above the upper landing surface.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about October 24, 2024, the employer did not ensure employees engaged in installing shingles on a 3/12 pitched residential roof located at 6483 Bayberry Drive, SEVEN HILLS, OH 44131 , 10.2 feet above the ground were protected by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring.  J.R. Roofing & Siding, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1468312 citation number 2, item number 1 and was affirmed as a final order on 05/14/2020, with respect to a workplace located at Site 30 Village Ln, BRECKSVILLE, OH 44141.
