29 CFR 1926.1417(a): The employer did not comply with all manufacturer procedures applicable to the operational functions of equipment, including its use with attachments. On or about October 20, 2024, and times prior thereto, near tower F6, employee(s) performed reconfiguration work without ensuring the rack pins for connecting the VarioTray to the erections rack as required by the manufacturer were in place.

29 CFR 1926.1423(e)(2):For non-assembly/disassembly work, the employer must provide and ensure the use of fall protection equipment for employees who are on a walking/working surface with an unprotected side or edge more than 6 feet above a lower level when employee(s) are at a work station on any part of the equipment (including the boom, of any type), except when the employee is at or near draw-works (when the equipment is running), in the cab, or on the deck. On or about October 20, 2024, and times prior thereto, near tower F6, employee(s) performed crane reconfiguration work without the use of a fall protection system from a working platform that had an unprotected edge more than six (6) feet above the lower level.
