29 CFR 1926.1403(a): Manufacturer procedures applicable to assembly and disassembly were not complied with: On or about May 25, 2023, at Tower #119 in Dimmit, Texas, where specific manufacturer procedures for the Liebherr Mobile Crane LTM 1750-9.1 prohibiting the touching of rotating rope pulleys during operations were not followed in this particular instance, exposing an employee to caught-in hazards.

29 CFR 1926.1404(d)(1)(iii): Before commencing assembly/disassembly operations, the A/D director did not ensure that the crew members understood all of the hazardous positions/locations to be avoided: On or about May 25, 2023 employees of CSI assembling a luffing crane to perform maintenance on a wind tower were exposed to caught-in hazards due to the employer not ensuring an employee who was working near a winching operation maintained clearance.
