Jobsite: 4737 Darrah Street a.)  On or about October 25, 2024 the employer failed to ensure that the flexible cord used to charge the powered industrial trucks was free from damage.   CSHO observed the flexible power cord wrapped in electrical tape.  Employes were required to handle the cord to charge the powered industrial truck.
