29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: At the job site located at 1804 East Village Estates Dr. Park City, Kansas, employees engaged in residential roofing activities from a 6:12 pitch roof were exposed to fall hazards of approximately 9 feet 1 inch from the eave of the roof to the ground below in that the extension ladder used to access the roof was not extended at least 3 feet above the eave or was not secured to the structure.
