Located at Building 5, 3197 Weeping Willow Drive, Reynoldsburg, Ohio, 43068: On or about October 30, 2024, two employees were working off a scaffold platform that was made from a stack of OSB sheathing that was elevated on the forks of a JLG G9-43A, Rough Terrain Telescope Forklift.  This condition exposed these employees to a fall hazard of approximately 24 feet to the ground level.

Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.  a. Located at Building 5, 3197 Weeping Willow Drive, Reynoldsburg, Ohio, 43068: On or about October 30, 2024, the employer failed to ensure that two employees working off a makeshift scaffold platform while installing roofing materials were protected from falls. This condition exposed the two employees to a fall hazard of up to approximately 24 feet to the ground level.
