29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about November 1, 2024, at 3860 Green Meadows Drive, Oak Creek, WI 53154, employees installing a roof on an existing residential home with an approximately 8:12 pitch roof were not using fall protection. This exposed employees to a fall hazard of approximately 12 feet 9 inches from the roof eve to the ground below on the back (North) side of the home.

(a) On or about November 1, 2024, at 3860 Green Meadows Drive, Oak Creek, WI 53154, employees were accessing the roof to conduct roofing work from a Gorilla ladder (front of home) which did not extend at least three feet above the upper landing surface.
