29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Jobsite, 808 Carbon Road, Greensburg, PA 15601: On or about October 18, 2024, procedures were not developed, documented and utilized for the control of potentially hazardous energy to protect employees during the emptying of a hopper for a Fielding Press. An employee was required to use a portable ladder to climb into a danger zone of the material handling system to visually determine how much material was in the trolley hopper. The trolley hopper system was not locked and tagged out to prevent it from moving while the employee was positioned near the tracks of the trolley. The employee was exposed to struck-by and crushed-by hazards.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: a) Jobsite, 808 Carbon Road, Greensburg, PA 15601: On or about October 18, 2024, an employee who was required to place part of his body into a dangner zone of a trolley hopper system on a Fielding Press had not been trained in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation. The employee was not trained as an Authorized employee under the employer's lock out tag out program.
