29 CFR 1910.147(c)(4)(i):  Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   a) Jobsite, 808 Carbon Road, Greensburg, PA 15601:   On or about October 18, 2024, procedures were not developed, documented and utilized for the control of potentially hazardous energy to protect employees during the emptying of a hopper for a Fielding Press.  An employee was required to use a portable ladder to climb into a danger zone of the material handling system to visually determine how much material was in the trolley hopper.  The trolley hopper system was not locked and tagged out to prevent it from moving while the employee was positioned near the tracks of the trolley.  The employee was exposed to struck-by and crushed-by hazards.
