1926.20(b)(2) The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s) Location: Worksite at Rt 106 East Bound off Route 24, WEST BRIDGEWATER, MA 02379 On or about 11/06/2024, the employer on scene did not perform an inspection before the start of work or during to ensure employees were protected from cave-ins while working in a trench/excavation at depths of 5 feet to approximately 8 feet deep. The employer did not have a program to conduct and document frequent and regular inspections of theor worksites to ensure that workers were protected from hazards of cave-ins.
