29 CFR 1910.261(k)(11): Steam and hot-water pipes. All exposed steam and hot-water pipes within 7 feet of the floor or working platform or within 15 inches measured horizontally from stairways, ramps, or fixed ladders shall be covered with an insulating material or guarded in such manner as to prevent contact. On or about October 30, 2024 and thereafter, at an establishment located at 1919 South Broadway, Green Bay, Wisconsin; (a) Paper Machine 9 Dry Side Mezzanine Level, the 35-pound steam pipe at the connection flange for the steam shower had an exposed steam pipe without insulating material exposing employees to thermal burns. (b) Paper Machine 9 Dry Side Mezzanine Level, the 35-pound steam pipe for Steam Shower, next to 35-pound steam supply to steam box bleed off valve had an exposed steam pipe without insulating material exposing employees to thermal burns. (c) Paper Machine 9 Drive Side Yankee Mezzanine Level, the 400-pound steam flange and bolts on the drive side mezzanine by the step down had an exposed steam pipe without insulating material exposing employees to thermal burns. (d) Paper Machine 9 Drive Side Yankee Mezzanine Level, the 400-pound steam pipe on the drive side mezzanine labeled "1-009 -003 Pressure" had an exposed steam pipe without insulating material exposing employees to thermal burns.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) shall be visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). Cord and plug connected equipment and flexible cord sets (extension cords) which remain connected once they are put in place and are not exposed to damage need not be visually inspected until they are relocated. On or about October 30, 2024 and thereafter, at an establishment located at 1919 South Broadway, Green Bay, Wisconsin; on the Drive Side Yankee Mezzanine Level for Paper Machine 9, the extension cord and Dayton fan electrical cord were not visually inspected before use on any shift for external defects such as damaged female plug and male prongs exposing employees to electrical shock and fire hazards.

On or about October 30, 2024 and thereafter, at an establishment located at 1919 South Broadway, Green Bay, Wisconsin; Paper Machine 9 Yankee Dryer Mezzanine Tender Side, the extended air nozzle attached to the red hose used to blow down and clean around the Yankee dryer had a pressure greater than 30 psi exposing employees to injuries related to compressed air entering the body such as swelling, severe pain, or air embolism.
