29 CFR  1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.  a. On or about October 1, 2024, employees were exposed to machine hazards associated with moving parts, in-going nip points, and pinch points during blade changes on the line 1103 Gamma Machinery bag machine (Model: 941BSV; S/N: 400B-1B), located in the production area. The employer failed to ensure employees utilized the developed energy control procedures to control the hazardous energy of the machine prior to performing the activity.   b. On or about October 7, 2024, and at times prior, employees were exposed to machine hazards associated with moving parts, in-going nip points, and pinch points during blade changes, rubber changes and tape changes on the line 1103 Gamma Machinery bag machine (Model: 941BSV; S/N: 400B-1B), located in the production area. The employer failed to ensure employees utilized the developed energy control procedures to control the hazardous energy of the machine prior to performing the activities.  c. On or about November 3, 2024, employees were exposed to machine hazards associated with rotating parts and pinch points during rethreading on the Gamma Machinery bag winder machine (Model: 118HD; SN: 3040-12). The employer failed to ensure employees utilized the developed energy control procedures to control the hazardous energy of the machine prior to performing the activity.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedure) was not done in sequence as required by 29 CFR 1910.147(d)(1) through (d)(6):  a.     On or about October 1, 2024, employees were exposed to machine hazards associated with moving parts, in-going nip points, and pinch points during blade changes on the line 1103 Gamma Machinery bag machine (Model: 941BSV; S/N: 400B-1B), located in the production area. The employer failed to implement energy control application steps as LOTO devices were not applied to the electrical disconnect [per the 1910.147(d)(4) requirements]. As a result, the remaining applicable energy control elements, involving dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.  b.    On or about November 3, 2024, employees were exposed to machine hazards associated with moving parts, in-going nip points, and pinch points during blade changes on the line 1103 Gamma Machinery winder  machine (Model: 118HD; S/N: 3064-11), located in the production area. The employer failed to implement energy control application steps as LOTO devices were not applied to the electrical disconnect [per the 1910.147(d)(4) requirements]. As a result, the remaining applicable energy control elements, involving dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.  c. 29 CFR  1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  a. On or about October 7 , 2024, the employer did not protect supervisors and employees from caught-in hazards by providing authorized employee lockout training related to energy control procedures necessary for performing servicing and/or maintenance tasks, such as, but not limited to, blade changes, rubber changes and tape changes on the Bag Machines.   b. On or about November 3, 2024, the employer did not protect supervisors and employees from caught-in hazards by providing authorized employee lockout training related to energy control procedures necessary for performing servicing and/or maintenance tasks, such as, but not limited to, rethreading on the Bag Winder Machines.

On or about October 9, 2024, the employer did not ensure that periodic inspections of the energy control procedures for the Gamma Machinery bag machines were conducted at least annually to ensure that the procedure and the requirements of the standard are being followed.
