29 CFR 1926.451(c)(2):Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation: a) Worksite was located at 710 Navesink River Road, Red Bank, NJ 07701: Subcontractor's workers were exposed to fall hazards of approximately fourteen feet to ground as they worked on replacing a roof from a ladder jack scaffold. The scaffold was set up with its base on the unstable debris pile of removed roofing materials. Violation was observed on or about November 7, 2024. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.1053(b)(6):Ladders were used on surfaces which were not stable and level: a) Worksite was located at 710 Navesink River Road, Red Bank, NJ 07701: Subcontractor's workers were working on replacing a roof and using an aluminum extension ladder to access the roof . The extension ladder was erected with its base on an unstable debris pile of removed roofing materials. Violation was observed on or about November 7, 2024. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: a) Worksite was located at 710 Navesink River Road, Red Bank, NJ 07701: Subcontractor's workers were exposed to fall hazards of approximately ten to thirty five feet to ground while they were tearing off the roof of an existing two - story single - family home without fall protection on. The lower roof elevation was a 4/12 rise and the upper roof elevation was a 10/12 rise. Violation was observed on or about November 7, 2024. All County Exteriors, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1560898 , citation number 1, item number 1 and was affirmed as a final order on January 6, 2022, with respect to a workplace located at 67 Metro Drive, Monroe Township, NJ 08831 . NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
