29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a. On or about November 8, 2024, at the above addressed jobsite, an employee working on the ground, under a worker performing residential roofing operations, was not wearing a hard hat to protect against falling materials and debris.  Vasat Exteriors, Carpentry and Roofing, Inc. was previously cited for a violation of this occupational safety and health standard 1926.100(a), which was contained in OSHA inspection number 1622955, citation number 1, item number 1 and was affirmed as a final order on November 21, 2022, with respect to a workplace located at 24 W. 560 77th St., NAPERVILLE, IL 60565.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. On or about November 8, 2024, at the above addressed jobsite, employees performing residential roofing operations, eighteen (18') feet nine (2") inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.  b. On or about November 8, 2024, at the above addressed jobsite, employees performing residential roofing operations, eight (8) feet eight (8) inches above the ground, were not protected from falls, thereby exposing the employees to fall hazards.  Vasat Exteriors, Carpentry and Roofing, Inc. was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1675968, citation number 1, item number 1 and was affirmed as a final order on October 12, 2023, with respect to a workplace located at 18482-18470 Bellagio Cir., in Tinley Park, Illinois.  Vasat Exteriors, Carpentry and Roofing, Inc. was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1622955, citation number 2, item number 1 and was affirmed as a final order on November 21, 2022, with respect to a workplace located at 24 W. 560 77th St., NAPERVILLE, IL 60565.  Vasat Exteriors, Carpentry and Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 1599541, citation number 1, item number 1, and was affirmed as a final order on August 8, 2022, with respect to a workplace located at 1240 S. Dement Rd., ROCHELLE, IL 61068.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.  a. On or about November 8, 2024, the portable ladder used to access the roof did not extend at least three (3') feet above the upper landing surface nor was it secured at the top, thereby exposing employees to the hazards associated with falls.  Vasat Exteriors, Carpentry and Roofing, Inc. was previously cited for a violation of this occupational safety and health standard 1926.1053(b)(1), which was contained in OSHA inspection number 1622955, citation number 1, item number 2 and was affirmed as a final order on November 21, 2022, with respect to a workplace located at 24 W. 560 77th St., NAPERVILLE, IL 60565.
