The employer did not retrain an employee when the employer had reason to believe the employee did not have the understanding and skill required by paragraphs (a) and (b) of this section. Situations requiring retraining include, but are not limited to, the situations listed in 29 CFR 1910.30(c)(1)-(3). a) Job Site: 521 North Center Ave, New Stanton, PA 15672: On or about November 12, 2024 the employer did not ensure employees had a safe means of egress to access the PF2 1 work platform. Employees of the maintenance department would use fixed equipment as a means of egress to climb down from the PF2 1 conveyor in order to access the work platform in order to perform maintenance on the drive motor.

29 CFR 1910.28(b)(1)(i):Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i): a) Job Site: 521 North Center Ave, New Stanton, PA 15672: On or about November 12, 2024 the employer did not ensure employees were protected from fall hazards while working on the PF2 1 work platform. Employees of the maintenance department were exposed to fall hazards of approximately 25 feet while traversing from the PF2 1 conveyor in order to access the work platform in order to perform maintenance on the drive motor.
