29 CFR  1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a) J & S Stone, LLC dba Northern OH Cut Stone LLC located at 16222 CR 34, Bellevue, Ohio, 44811: On or about November 6, 2024, Employees tasked with performing servicing or maintenance activities such as, but not limited to, changing blades and set up activities on the Ty-sa-man  and Predator industrial masonry saws in Building 1 without the benefit of energy isolation, hazardous energy recognition training and employer evaluation, exposed employees to amputation hazards created by the unexpected energization of the equipment.

29 CFR  1910.178(l)(2)(ii):The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.  a) J & S Stone, LLC dba Northern OH Cut Stone LLC located at 16222 CR 34, Bellevue, Ohio,44811: On or about November 16, 2024, the employer failed to ensure that each employee tasked with operating powered industrial vehicles such as, but not limited to the Clark (model CGP23) forklift were adequately trained through a combination of formal instruction, practical training, and evaluation prior to operating the vehicles throughout the facility.  Powered industrial vehicle operators or pedestrians were exposed to potential struck-by and crushing hazards in that employees operated the vehicles without the benefit of training.
