1926.454(a)(5) The employer did not train each employee who performs work on a scaffold, by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards, including any other pertinent requirements of this subpart (a) Quarryville Water Tank - Employees worked about two bucks down from the top of the over 100-foot high fabricated frame scaffolding erected around the exterior of the cylindrical shaped water tank to install sheeting around the scaffold's outer perimeter. This working level did not have full planking. This working level had only two 2x10 scaffold planks at the working level's outer perimeter rather than five 2x10 planks needed to cover the total space between the uprights, on or about November 13, 2024. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

1926.454(c)(3) The employer failed to retrain employees that showed indications through work performance of not retaining the requisite proficiency: (a) Quarryville Water Tank - Employees worked about two bucks down from the top of the over 100-foot high fabricated frame scaffolding erected around the exterior of the cylindrical shaped water tank to install sheeting around the scaffold's outer perimeter. Next to the electrical box for the water tank at the northwest side of the water tank, a mud sill, consisting of six stacked boards under the base plate of a sectional frame upright, was uneven, sloping downward from south to north, on or about November 13, 2024. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

(a) Quarryville Water Tank - While disassembling the supported scaffolding erected around the exterior of the water tank, an employee, working on the very top section of the scaffolding, climbed down the frame using the cross braces on the inside as footing to get from the top level to the next lower level before continuing to disassemble, on or about February 4, 2025. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
