29 CFR  1910.22(a)(3):29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.  At a worksite located at 6530 Winner Road, Kansas City, MO 64125:   The employer failed to protect employees from slip and fall hazards. This was most recently demonstrated on or about December 19, 2024, and at times prior to when there was oil that was leaking onto the floor in the compressor room. Employee(s) had to enter the room to turn on and off the compressor equipment and thereby exposing employees to possible injury.

29 CFR  1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  At a worksite located at 6530 Winner Road, Kansas City, MO 64125:   The employer failed to protect employees from injurious from corrosive materials. This was most recently demonstrated on or about December 19, 2024, and at times prior to when wall-mounted eyewash stations were not in working order. Employee(s) use various chemicals as part of the manufacturing process at the facility, and some of the materials are corrosive. In the event of contact with the eye or skin, a lack of drenching or flushing could lead to further injury.

29 CFR 1910.243(b)(2): Air hose and hose connections used for conducting compressed air to utilization equipment were not designed for the pressure and service to which they were subjected.  At a worksite located at 6530 Winner Road, Kansas City, MO 64125:   The employer failed to protect employees from struck-by hazards. This was most recently demonstrated on or about January 28, 2025, and at times prior to when there was an air hose that was used in part to supply air to pneumatic air lines. The air hose(s) were attached to the hose fitting by a worm drive-type hose clamp which was not designed for use on an air hose. The clamps were located throughout the facility and associated with various pieces of equipment and hand held power tools and thereby exposing employees to possible whipping action of the hose if the clamp were to come off.

29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed:  At a worksite located at 6530 Winner Road, Kansas City, MO 64125: :   The employer failed to protect employees from exposure to potential physical and health-related hazards. This was most recently demonstrated on or about December 19, 2024, and at times prior to, when workers were exposed to physical and health-related hazards; including but not limited to flying wood particles, falling wood boards or large pieces, liquid chemicals, wood dust, and noisy equipment in the workplace as part of the manufacturing process. The employer had not verified that a hazard assessment had been performed at the facility through a written certification.
