29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. a) The Flats Building, 1 Culvert Drive, Link @ W Squared, West Windsor NJ: Company worksite employees were exposed to fall hazards, in excess of 13 feet and 19 feet 5 inches to the ground surface, when working/walking at/from unprotected supported scaffold's end frame openings without fall protection. Observed on or about 11/20/24. b) The Flats Building, 1 Culvert Drive, Link @ W Squared, West Windsor NJ: Company worksite employees were exposed to fall hazards, in excess of 19 feet 5 inches to the ground surface, when working/walking from a supported scaffold whose walking/working surface was not supplied with mid rails. Observed on or about 11/20/24. c) The Flats Building, 1 Culvert Drive, Link @ W Squared, West Windsor NJ: A company worksite employee was exposed to fall hazards, in excess of 13 feet to the ground surface, when working/walking from a supported scaffold whose walking/working surface was not supplied with mid rails. 29 CFR 1926.451(g)(3)(i): Vertical lifelines were not fastened to a fixed safe point of anchorage, independent of the scaffold, and protected from sharp edges and abrasion. a)The Flats Building, 1 Culvert Drive, Link @ W Squared, West Windsor NJ: The anchor point used for an employee's Personnal Fall Arrest system was a structural member within an end frame of a prefabricated, tubular welded framed scaffold's fourth buck exposing this employee to crushing and fall hazards (in excess of 18 feet 7 inches to the ground surface below). Observed on or about 11/20/24.
