29 CFR 1926.100(a): Employees were not provided protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  a) On November 22, 2024, employees, who were working below other employees performing roofing work, were exposed to struck-by hazards in that they were not wearing hard hats.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR  1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) On November 22, 2024, employees engaged in roofing work at 20 St. Thomas Colony, Fox Lake, IL, were not protected from fall hazards while working at elevations approximately 15.7 feet above the ground.  b) On November 22, 2024, employees engaged in roofing work at 23 Jamaica Colony, Fox Lake, IL, were not protected from fall hazards while working at elevations approximately 17 feet above the ground.  Premium Home Experts Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1762519, citation number 1, item number 1 and was affirmed as a final order on October 8, 2024, with respect to a workplace located at 1178 - 1184 Parkside., Palatine, IL 60038.  Premium Home Experts Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1667693, citation number 2, item number 1 and was affirmed as a final order on September 6, 2023, with respect to a workplace located at 586 W Parkside Dr., Palatine, IL 60067.  Premium Home Experts Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1581564, citation number 1, item number 1 and was affirmed as a final order on May 24, 2022, with respect to a workplace located at 105 W Lincoln Hwy., Dekalb, IL 60115.   No abatement certification or documentation is required for this item.
