29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b)  a) On the roof of building 1 on the site located at 14810 SR 64 E, Bradenton, FL: On or about 11/25/2024, the employer exposed employees to fall hazards, in that, fall protection was not utilized by employees while installing sheathing on the roof of a residential roof that had a fall height of approximately 41 ft. 2 in. to the ground.

29 CFR  1926.502(b)(2):Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high.  a) On the third floor of building 1 on the site located at 14810 SR 64 E, Bradenton, FL: On or about 11/25/2024, the employer exposed employees to fall hazards, in that, the guardrails around a hole in the floor where stairs were being added was missing a midrail.
