FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

Employees were not provided information and were not trained in the hazards associated with Methylene Chloride and its exposure risks, on or about 12/2/25.
